Amgen Australia Pty Ltd (Amgen) has contracted Imagine If Health Pty Ltd (Imagine If) to provide this portal (www.amgen medicationrequests.com.au) and coordination services to manage Amgen’s Medication Request program.
Amgen and Imagine If comply with the Privacy Act 1988 (Cth) (“Privacy Act”) to ensure that your personal information is protected. In this statement, all references to personal information include sensitive information (such as information about an individual’s health details).
Amgen and Imagine If will collect, hold and use your personal information (gathered during enrolment) and information on individual patients to assess, manage, and deliver requests for medications covered by the Imagine If system and to continue to improve the Program, including contacting you for follow-up purposes.
If you provide your email address, telephone and/or mobile phone number in this system, you also consent to Amgen and Imagine If using your email address, telephone and/or mobile phone number to contact you (including by SMS or email) for any of the above purposes. Your consent will remain current until you advise us otherwise by writing to us at the relevant address below.
Amgen continually monitor the safety profile of their products. To support this ongoing process, and to adhere with local and global regulatory obligations, details of any potential reports of Adverse Events (AE), special situations and product quality complaints, in relation to a patient potentially administered an Amgen product, identified as part of your participation in this activity, will be shared with Amgen’s medical (pharmacovigilance) department. The information may be disclosed to local and overseas regulatory authorities or other third parties (such as those providing AE case processing activities, or license partners), for the purposes of meeting pharmacovigilance requirements.
Amgen’s medical (pharmacovigilance) team may wish to contact you, or your nominated Healthcare Professional (HCP), for further information regarding any adverse events, special situations and product quality complaints, identified during your participation in this activity. Your agreement to this is voluntary, and you will be given the option to indicate your consent to this during the provision of the program services.
Amgen and/or Imagine If may transfer your personal information overseas. In the event that Amgen and/or Imagine If transfer your personal information outside Australia, Amgen and/or Imagine If will comply with the requirements of the Privacy Act that relate to transborder data flows.
Unless you inform us otherwise, by submitting this form, you consent to Amgen and Imagine If collecting, using and disclosing your personal information in this way. If you do not provide the information to us, we will be unable to assess and manage requests for medication in this portal.
